Title:                     Case Manager

Program:             Fort Washington – Team Pride

 

Program Description

The Fort Washington Men’s Shelter is a 200 bed temporary shelter which Project Renewal manages under a contract with the Department of Homeless Services and the Department of Health and Mental Hygiene. Comprehensive psychiatric, medical and case management services are provided to homeless men who have been diagnosed with mental illness and substance abuse. 

 

The Program Director is specifically responsible for the On Site Rehabilitation Services consisting of two approaches-Team PRIDE and Team STEPS. These programs provide assessment and case management services targeted to reduce the length of stay in the shelter and facilitate the transition to community based housing. With a focus on housing placement, individual and group counseling is used to assist clients in enhancing their adult living skills, and connecting to community based healthcare, mental health and substance abuse services. 

 

Overall Responsibilities

Under the general direction of the Program Director, the Case Manager is responsible for providing the required assessment, case management, and counseling services to the selected clients to make them able to address their dependencies and assume control of their lives to ultimately live independently and return to permanent or other alternative housing in their communities.

 

Essential Duties and Responsibilities

The essential duties of the Case Manager include but are not limited to the following activities: 

·         Provides case management for a caseload of approximately 25 men.

·         Conducts intake and psychosocial and substance abuse assessments of clients.

·         In collaboration with the client, prepares and monitors the Individual Case Management Plan for each client; identifying short and short-term objectives to be achieved.

·         Counsels clients through individualized and group sessions on issues such achieving personal goals, finding permanent or alternative housing, maintaining sobriety, medication compliance and employment opportunities.

·         Identifies services and makes referrals of clients for medical care, substance abuse services, and employment services.

*Available to respond to emergencies on a 24/7/365 basis, especially after normal work hours and on weekend*

 

Education Requirements

·         All candidates must have at least a Bachelor’s degree from an accredited college or university. 

·         A Master’s degree in Social Work or other human services field is strongly preferred.

·         A minimum of three (3) years responsible experience working with mentally ill individuals or homeless individuals with either a diagnosis of mental illness or a substance addiction

·         Knowledge of case management softwares as well as a proficiency in Microsoft Office Suite.

Program Description

Geffner House is a 307 unit supportive Single Room Occupancy (SRO) building that provides permanent housing for low income men and women, including people with serious and persistent mental illness; individuals who are recovering from drug or alcohol addiction; and people who are living with HIV/AIDS. Tenants are offered a comprehensive array of services. 

 

Essential Duties and Responsibilities

The essential duties of the Case Manager include but are not limited to the following activities:

·         Provides intake, psychosocial and substance abuse assessments to tenants and develops an appropriate treatment plan for each tenant in the assigned caseload.

·         Makes home visits, coordinates medical, psychiatric and substance abuse services.

·         Monitors medication compliance as necessary.

·         Maintains up to date written information or case notes on contacts with the tenants and any other required information, and enters all contacts and interventions into the FOOTHOLD/AWARDS system. 

·         Conducts individual and group supportive counseling sessions.

·         Organizes and educates tenants through groups and other activities presenting issues of interest to the tenants such as money management, employment opportunities etc.

·         Links tenants to community services.

 

Education/Skills Requirements

·         A Bachelor’s degree from an accredited college in social work, psychology or related human services

  • A Master’s degree in social work, psychology, or related human services field preferred

  • A minimum of one (1) year of experience working with a special needs population, especially individuals with a history of homelessness and mental illness or substance addiction.

  • Knowledge of case management software, and proficiency in Microsoft Office Suite

  • Fluency in Spanish is a plus.

Intensive Case Manager

Title:                     Case Manager

Program:             Parole Support & Treatment Program; Permanent Housing Program

 

Program Description:

Project Renewal’s Re-Entry Permanent Housing provides supported housing to 45 men/women with serious and persistent mental illness/dual diagnoses – who also have a history of significant criminal justice involvement/may be under parole supervision.  The program directly delivers and/or facilitates access to a broad range of services geared towards clients’ stabilization and community integration.

 

Overall Responsibilities

Under the general direction of the Program Coordinator, the Case Manager is responsible for assessing clients to determine what is needed to stabilize them, and working with them to create individual plans to address these needs so they can achieve successful community re-entry.

 

Essential Duties and Responsibilities:

The essential duties of the Case Manager include but are not limited to the following activities:            

·         Conducts assessments of clients entering the program and assists them in developing individualized Housing Support/Service Plans.

·         Reviews the Housing Support/Service Plan every 6 months- or as needed.

·         Makes face to face contact with each client at a minimum 2 times each month.

·         Facilitates clients’ access to community based treatment and support services – which may include escort and direct linkage.

·         Maintains up to date documentation of all contacts in CAIRS and AWARDS.

·         Participates in regular meetings with the permanent housing staff to discuss the status of each client in the program. 

·         Participates in on-call response system.

·         Performs other duties as assigned by supervisory staff.

 

Qualifications

·         Candidates must have a Bachelor’s Degree in Social Work or other Human Services related field with significant experience working with the seriously mentally ill, dually diagnosed Re-Entry population.  

·         A Master’s Degree is strongly preferred.

·         Proficiency in the Microsoft Office, especially Work and Excel and familiarity with case management software.

Title:               Violations and Safety Compliance Coordinator

Reports to:     Director of Facilities

 

Essential Duties and Responsibilities:

The essential duties of the Violations and Safety Compliance Coordinator include but are not limited to the following activities:

·         Coordinate responses to changing regulatory compliance and notices of violation from various agencies

·         Review, track and document corrections of violations cited and confirm appropriate responses are filed with citing agencies

·         Maintain up-to-date and accurate records of building violations, associated responses and corrections.

·         Coordinate and/or conduct walkthroughs of facilities to prepare for site inspections, helping to prioritize site preparations and staffing assignments 

·         Attend Environmental Control Board (ECB) hearings.

·         Manage work order system, tracking tasks performed, work orders completed and associated costs involved.

·         Ensure that maintenance policies and procedures are communicated to all PRI facilities staff

·         Support facilities department and building managers with training materials, documents and other communications.

·         Communicate and support workplace safety, assist with identifying and acquiring necessary tools and equipment for employees based on project needs and safety issues.

·         Travel to various PRI sites and locations

·         Other duties as assigned.

 

Education/Experience and Skills:

·         Minimum High School diploma or equivalent and 7 years work experience

·         Bachelor’s degree a plus

·         3 years’ experience in areas of maintenance and/or construction, ideally in a residential or shelter setting.

·         10-hour OSHA certificate (must be obtained within six months of start date)

·         Current, Valid Driver License which must be kept in good standing

·         Proficient in Microsoft Suite

·         Able to lift 25 pounds

Title:                     Residential Aide Supervisor

Program:             East Williamsburg Men’s Shelter

 

Program Description

East Williamsburg Men’s Shelter is an emergency men’s shelter which  helps 98- 140 individuals ages 55 and up with substance abuse histories work towards health, sobriety, and, ultimately, permanent housing and employment. In addition to lodging and three meals a day, clients receive comprehensive case management services, plus an array of substance abuse related and health care services provided by different programs; such as Medically Supervised Outpatient Withdrawal Services (MSOWS); Chemical Dependency Crisis Service (CDCS), and Outpatient Recovery Center available to them throughout the agency.

 

Essential Duties and Responsibilities:

Depending upon the shift, the essential duties of the Residential Aide Supervisor include but are not limited to the following activities:

·         Provides guidance and direction to the Resident Aides ensuring adequate coverage for each shift; monitoring their performance and initiating administrative action as warranted.

·         Oversees the operation of the facility, including handling emergency and crisis situations during their tour of duty.

·         Provides orientation for new arrivals, informing them of shelter rules and regulations, giving them a tour of the facility, assigning a locker and a bed, and giving them their basic supplies, including sheets, towels and toiletries.   

·         Oversees scheduled removal of linens by the Residential Aides, ensuring that proper protective protocols are followed and that the linens are properly bagged for pick up by the vendor contracted to launder them.

·         Assist the weekend and evening clinical staff in getting clients access to their medications and responding to client issues.

·         Ensures that any incident is recorded in the Log Book, but especially that priority incidents are called in to the Shelter Director and Assistant Shelter Director and that documentation is subsequently provided to supervisory staff in a timely manner.

·         Performs escort duties or ensures escorts are provided to clients as requested by the Case Managers.

·         Participates in some of the recreational activities offered to the clients, encouraging them to actively participate in order the gain the benefits of the activity.

·         Distributes to clients any MetroCards received from the Social Services staff.

 

Education Requirements

·         Associate Degree plus experience as outlined in item 1 below, or

·         High School Diploma or GED plus experience as outlined in item 2 below.  

·         Minimum of one (1) year of experience working with people diagnosed with mental illness or having addiction or substance abuse issues. 

·         Minimum of two (2) years of experience working with people diagnosed with mental illness or having addiction or substance abuse issues.  

·         Knowledge of Microsoft Office Suite.

·         Bilingual or multilingual a plus.

Program Description

Ana’s Place is a 108 bed 24/7 emergency shelter for homeless men diagnosed with mental illness and/or substance abuse.  Our comprehensive services include case management, individual and group counseling, recreational activities and housing placement assistance.  Additionally, an on-site psychiatric team provides services Monday through Friday, staffed by a psychiatrist, nurse practitioner, registered nurse, and a licensed practical nurse.

 

Essential Duties and Responsibilities

The essential duties of the Operations Shift Supervisor include but are not limited to the following activities depending on the on the particular tour of duty:

·         Communicates with clinical staff regarding any shift issues affecting the clients, by conducting a comprehensive review of log book and staff observations, recording any changes in behavior of the clients that might be useful to the team.

·         Participates in weekly social service meetings as Liaison to operations staff, debriefing any incidents which may need more information.  

·         Conducts supervisory meetings with all RA supervisors to review procedures and maintain that communication is being consistent across all shifts.

·         Ensures that any and all incidents are recorded in the Log Book, but especially that priority incidents are recorded, and the appropriate management staff is notified.

·         Reviews incidents with staff to ensure quality control, reporting any inconsistencies to Assistant Director.

·         Ensures that electronic shift summaries are completed and all incident reports are inputted coherently.

·         Oversees the client storage room and regularly reviews records to ensure accuracy/consistency in documentation

·         Oversees the Callahan room to ensure all records are up-to-date and all supplies are accounted for.

·         As needed, provides direct assistance to clients can which include helping clients adhere to their daily schedules, and ensuring they are participating in their scheduled activities. 

 

Education Requirements

·         A High School Diploma or GED

·         A minimum of two years of experience working with people diagnosed with mental illness or having a substance abuse addiction. One year of the required experience must have been in a supervisory capacity.

·         Must have a CPR and Certificate of Fitness for a Fire Guard, F-80, or have it completed within 90 days of starting employment.

·         Knowledge of Microsoft Office Suite, especially e-mail, WORD, and EXCEL.

·         Provide clerical support to the expanded Social Services Department.

·         Assist in collecting data for new DHS initiatives

Title:                     Psychiatry Provider

Program:             Marsha’s House, New Providence and Ana’s Place

Overall Responsibilities:

Psychiatry providers will conduct comprehensive evaluations, medication management and crisis intervention for homeless men and women living in a shelter. Providers will collaborate with shelter staff as well as liaison with outside providers as appropriate. Positions offer opportunity for administrative responsibilities.

Essential Duties and Responsibilities:

The essential duties of the Nurse Practitioner/Psychiatrist include but are not limited to the following activities:               

  • Oversees all psychiatric services at facility

  • Provides psychiatric evaluations for individuals referred for assessment of psychiatric conditions or for entitlements and housing

  • Provides psychiatric follow-up care as needed, including psychopharmacology and supportive psychotherapy

  • Adheres to agency policy regarding obtaining health screening and laboratory monitoring

  • Provides crisis intervention and assessment as needed

  • In collaboration with on-site nurse, provides liaison to outside psychiatric and medical providers as necessary and appropriate

·         Completes documentation in compliance with agency policies and procedures

  • Participates in weekly clinical team meeting. Provides case consultation to interdisciplinary staff

  • Provides in-service training to staff on psychiatric disorders, addiction, and other topics as needed

  • Oversight of licensed practical nurse, including medication system

  • Provides collaborative agreement for nurse practitioner(s) working within programs

Required Credentials

·         Current New York State medical license and registration

New Providence Women’s Shelter is a 130 bed 24/7 emergency shelter for homeless women diagnosed with mental illness and/or substance abuse.  Our comprehensive services include case management, individual and group counseling, recreational activities and housing placement assistance.  Additionally, an on-site medical and psychiatric clinic operates Monday through Friday, staffed by a nurse, primary care physician and two psychiatric nurse practitioners.

Essential Duties and Responsibilities

The essential duties of the Team Leader include but are not limited to the following activities:

·         Performs a thorough assessment of clients being referred to the shelter to determine their appropriateness for our setting within a 24 hour time period and assign to Case Manager.

·         Provide guidance and direct supervision to an assigned team of Case Managers to ensure assessments are performed.

·         Assists in quality assurance activities to ensure service milestones/program performance goals are met.

·         As part of the 6 month after care services, monitors status of clients placed in the community to ensure successful community integration.

·          Facilitates the provision of any assistance needed to ensure that clients remain housed.

·         Ensures that accurate and up to date clinical documents are being maintained and that required data is being inputted in a timely fashion into the CARES system by the Case Managers.

·         Attends and participates in the weekly Case Conference meetings on the status of each client

Education and Skills Requirements

·         Master’s Degree in Social Work (LMSW/LCSW preferred but not required)

·         A minimum of four years of experience working with homeless Mentally Ill and Chemical Affected also known as (MICA) individuals or other special needs populations.

·         The ideal candidate will have at least two years of supervisory experience.

·         Knowledge of case management software’s, as well as a proficiency in Microsoft Office Suite

Overall Responsibilities:

Under the supervision of the Director of Mental Health Housing, the Program Director will be responsible for the oversight of day-to-day operations including all aspects of service delivery and staff supervision.

·         Provides guidance and direction to 2 Case Managers and 1 Independent Living Specialist in engaging and assessing clients and creating individual housing support plans.

·         Identifies needs and coordinates ongoing training for staff.

·         Builds and maintains strong connections with both local and state service providers.

·         Ensures timely completion of statistical and reporting requirements.

·         Collaborates with the Director of Mental Health Housing to implement and enforce policies/procedures that adhere to PRI internal standards and the monitoring agency’s requirements.

·         Identifies and secures apartments within the community and maintains liaisons with building superintendents and property management to ensure that tenant health and safety requirements are upheld.

·         24-hour on call coverage for client related emergencies.

·         Other tasks as assigned.

 

Qualifications:

·         Master’s degree in Social Work or other Human Services related field preferred with 1-2 years of experience working with the seriously mentally ill, dually diagnosed, homeless adults;

·         Or a Bachelor’s degree in Social Work or other Human Services related field with a minimum 5 years of experience working with the seriously mentally ill, dually diagnosed, homeless adults.

·         Two years of experience in a supervisory role is required.

·         An ability to communicate effectively both orally and in writing and to listen constructively to staff and clients.

·         Responsible for providing a limited creative arts therapy program for Mentally Ill Adults with a history of chronic homelessness

·         Ensures the program is varied and engaging, meeting the psychological, cognitive, physical, functional, social, and expressive needs of consumer’s while complying with OMH 595 regulations.

·         Monitors and evaluates the effectiveness of individual and group creative arts therapy interventions and activities, modifying as necessary to achieve goals and improve performance. 

·         Builds and maintains positive, collaborative relationships with interdisciplinary team members, integrating goals and plans to promote resident care, safety, and satisfaction.

·         Licensed to practice as a Creative Arts Therapist in New York State;

Program Description

Clinton Residence is a transitional treatment facility licensed and regulated by the NYS Office of Mental Health.  The facility provides transitional housing for 57 formerly homeless, single adults with chronic mental illness, some of whom are dually diagnosed with a substance abuse disorder.

 

Overall Responsibilities:

Under the direction of the Building Superintendent, the Custodian is responsible for assisting in the maintenance of the residential building. 

 

Essential Duties and Responsibilities

The essential duties of the Custodian include, but are not limited to the following activities:

·         Responsible for cleaning and disinfecting all commons areas and offices, including bathrooms, stairwells and hallways, to ensure that the building is kept clean

·         Assist in preparing apartments for new residents and with move in/ out process

·         Assist Building Super as directed in minor repairs and correcting potential safety hazards as needed

·         Empty trash and garbage containers and prepare refuse for pick-up

·         Keep building exterior in safe and presentable condition, including ice/ snow removal; sidewalk maintenance and central garden/ courtyard area

·         Under supervision of Building Super, develop building-wide knowledge of all mechanical system operation and maintenance

·         Assist in documentation of fire drills smoke detector/ CO2 detector maintenance

·         Performs other duties as assigned by the Program Director or Clinical Director

 

Education Requirements

·         A High School diploma or GED is required.   

·         One year of experience working in a residential program serving individuals with mental illness and chemical addictions preferred.

·         Maintains clinic appointments for patients.

·         Assists with patient flow.

·         Maintains all clinic files and equipment.

·         Orders supplies and materials.

·         Enters all necessary billing data and maintains accurate patient data base.

·         Serves as liaison with case managers, medication nurse, and psychiatrists.

 

Qualifications:

·         High School Diploma or Equivalency (Required)

·         Associates Degree (preferred)

·         Prior healthcare experience preferred

·         Basic keyboarding and computer literacy

·         Bilingual (Spanish/English) required
